Изучить КТ-семиотику внутрипеченочной холангиокарциномы (ВПХК) для определения прогностических маркеров рецидива и проанализировать наличие ассоциативных связей между КТ-характеристиками ВПХК и мутациями в генах IDH1/2, MET, KRAS, BRAF, ERBB2, EGFR, FGFR.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведен анализ баз данных и диагностических изображений НМИЦ хирургии им. А. В. Вишневского и Московского клинического научного центра им. А. С. Логинова за период с апреля 2016 по январь 2022 гг. по ключевым запросам «внутрипеченочная холангиокарцинома», «печень», «гепатоцеллюлярная карцинома», «метастазы», «радиогеномика». Идентифицировано 142 пациента с новообразованиями печени, включая 90 случаев ВПХК, 31 случай гепатоцеллюлярной карциномы и 21 случай метастатического поражения печени, все морфологически верифицированные (гистологический и иммуногистохимический анализ биопсийного материала).</p></sec><sec><title>Результаты</title><p>Результаты. Определяются ассоциативные связи между КТ-признаками и мутациями генов MET и IDH1/2. По результатам статистического анализа все четыре КТ-признака, такие как дилатация желчных протоков, ретракция капсулы, наличие очагов отсева и изменения объемов тканей, прослеживают взаимосвязь с вероятностью возникновения рецидива или смерти у пациентов с наличием ВПХК.</p></sec><sec><title>Заключение</title><p>Заключение. В ретроспективном исследовании наши результаты подчеркивают потенциальную прогностическую значимость КТ-признаков ВПХК. Нами выявлены КТ-признаки, позволяющие провести дифференциальную диагностику ВПХК с гепатоцеллюлярной карциномой и метастазами колоректального рака. Также были выявлены ассоциации между КТ-признаками ВПХК и мутациями генов IDH1/2 и MET, что может в дальнейшем дать возможность неинвазивно получать данные о клинически значимых молекулярных маркерах опухолей для применения персонализированного подхода в лечении пациентов.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. To study the CT semiotics of intrahepatic cholangiocarcinoma (ICC) to determine the prognostic markers of recurrence. To analyze the association between CT characteristics of ICC and mutations in IDH1/2, MET, KRAS, BRAF, ERBB2, EGFR, FGFR genes.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. We analyzed databases and diagnostic images of Vishnevsky National Medical Research Center of Surgery and Loginov Moscow Clinical Research Center for the period from April 2016 to January 2022 using the key queries «intrahepatic cholangiocarcinoma», «liver», «hepatocellular carcinoma», «metastases», «radio genomics». 142 patients with liver neoplasms were identified, including 90 cases of ICC, 31 cases of hepatocellular carcinoma and 21 cases of metastatic liver lesions, all morphologically verified (histologic and immunohistochemical analysis of biopsy material).</p></sec><sec><title>Results</title><p>Results. Associations between CT features and mutations of MET and IDH1/2 genes were determined. According to the results of statistical analysis all four CT-signs, such as bile duct dilatation, capsule retraction, presence of dropout foci and tissue volume changes, are correlated with the probability of recurrence (death) in patients with ICC.</p></sec><sec><title>Conclusion</title><p>Conclusion. In a retrospective study, our results emphasize the potential prognostic significance of CT signs of ICC. We identified CT signs that allow differential diagnosis of ICC with hepatocellular carcinoma and colorectal cancer metastases.
