Выявление особенностей течения немелкоклеточного рака легкого (РЛ) и информативности процентного содержания лимфоцитов как показателя адаптационного статуса больных, перенесших COVID‑19, подтвержденный результатами ПЦР‑диагностики, до начала противоопухолевого лечения.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Были изучены клинико‑лабораторные показатели и лейкоцитарная формула первичных больных немелкоклеточным РЛ st. I–III, мужчин (59) и женщин (32) в возрасте 36–75 лет. Основные группы составили 32 мужчины и 16 женщин, перенесших COVID‑19, подтвержденный результатами ПЦР‑диагностики, за 2–9 мес. до госпитализации. Пациенты контрольных групп отрицали наличие в анамнезе диагностированного COVID‑19. Перед началом противоопухолевого лечения оценивали процентное содержание лимфоцитов в крови. Отмечали случаи прогрессирования РЛ и летальность в течение первого года после противоопухолевого лечения.</p></sec><sec><title>Результаты</title><p>Результаты. У мужчин основной группы было отмечено увеличение в 2,7 раз (p &lt; 0,05) случаев РЛ ранних стадий при одновременной тенденции к росту случаев метастазирования и летальности в течение года после лечения в стационаре по сравнению с показателями в контрольной группе. У женщин наблюдались признаки ухудшения течения заболевания и снижения эффективности лечения без заметных изменений соотношения показателей распространенности опухолевого процесса. У пациентов основных групп было отмечено нарушение соответствия между процентным числом лимфоцитов и распространенностью и динамикой злокачественного процесса. У мужчин основной группы, умерших в течение года после окончания лечения РЛ, процентное содержание лимфоцитов перед началом лечения достигало значений 28–45 %, соответствующих антистрессорным адаптационным реакциям, в 4 раза чаще, чем в контрольной группе (p &lt; 0,01).</p></sec><sec><title>Заключение</title><p>Заключение. COVID‑19, перенесенный до начала лечения РЛ, способствует снижению противоопухолевой резистентности организма и нарушению информативности процентного содержания лимфоцитов как показателя адаптационного статуса. Изменения зависят от пола. У женщин отмечены признаки усугубления злокачественного процесса и системных нарушений, снижение эффективности лечения. У мужчин резкий рост числа случаев годичной летальности при высоких значениях процентного содержания лимфоцитов перед началом лечения, а также числа случаев РЛ начальных стадий указывает на более значительное изменение состояния лимфоцитов по сравнению с имеющим место только при РЛ, а также на возможность ускорения перехода предопухолевого состояния в злокачественный процесс в легких под влиянием предшествующей короновирусной инфекции.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. This study aimed to identify the characteristics of the course of cell lung cancer (LC) and the information content of the indicators of the adaptation status of patients of different sexes who had COVID‑19, confirmed by PCR diagnostic results before the start of antitumor treatment.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. We have studied traditional clinical and laboratory parameters, and the white blood cell count in patients with non‑small cell LC st. I–III, men (59) and women (32) aged 36–75 years. The main groups included 32 men and 16 women who had COVID‑19, confirmed by real‑time PCR, 2–9 months before hospitalization. Patients in the control groups denied having a history of diagnosed COVID‑19. Before the start of antitumor treatment, the percentage of lymphocytes in the blood was assessed. Cases of LC progression and mortality within a year after antitumor treatment were noted.</p></sec><sec><title>Results</title><p>Results. In men of the main group a 2.7‑fold increase (p &lt; 0.05) in cases of early lung cancer stagings with a simultaneous tendency towards an increase in the number of incidences of metastasizing and mortality within a year after inpatient treatment in comparison with control indexes were observed. Signs of disease severity worsening and a decrease in the effectiveness of treatment were observed without significant changes in the ratio of the patients with tumors of various stagings in female patients. In patients of the main groups, a disturbance of the correspondence between the percentage of lymphocytes and the prevalence and dynamics of the malignant process was noted. In men of the main group who died within the first year after the end of antitumor treatment, the percentage of lymphocytes before the start of treatment reached 28–45 %, corresponding to antistress adaptational reactions, 4 times more often than in the control group (p &lt; 0.01)</p></sec><sec><title>Conclusion</title><p>Conclusion. COVID‑19 flow before the start of LC treatment contributes to a decrease in the body's antitumor resistance and a violation of the informativeness of the percentage of blood lymphocytes as an indicator of adaptation status. Changes depend on sex. In women, the observed changes reflected an aggravation of the malignant process and systemic disorders, as well as a decrease in the effectiveness of treatment.
