В представленном клиническом наблюдении продемонстрирована эффективность лимфовенулярного анастомоза (ЛВА) как метода, позволяющего устранить патологические процессы при обструктивной лимфедеме на ранних и средних этапах развития заболевания. Подчеркивается важность предоперационного обследования и картирования лимфатических и венозных сосудов с использованием ультразвуковых технологий, что позволяет оптимизировать тактику хирургического вмешательства, повысить его эффективность и сократить время проведения операции. Рассмотрены особенности патофизиологии лимфедемы, включая склероз сосудов и снижение их способности к лимфооттоку, а также преимущества современных методов инструментальной диагностики в планировании ЛВА. В статье акцентируется внимание на необходимости индивидуального подхода к пациентам, основанного на тщательном предоперационном исследовании сосудистого русла, а также на важности использования ультразвукового мониторинга в послеоперационном периоде для контроля состояния при лимфедеме.</p><p>Таким образом, определяющее значение комплексной стратегии, включающей в себя предварительную диагностику и тщательное планирование, обеспечивает наиболее эффективные клинические исходы при лечении лимфедемы верхних конечностей методом ЛВА.</p></abstract><trans-abstract xml:lang="en"><p>Upper limb lymphedema following axillary lymph node dissection for breast cancer is one of the most common postoperative complications, significantly reducing patients’ quality of life. This clinical case demonstrates the effectiveness of lymphovenular anastomosis (LVA) as a method that allows for the correction of pathological processes associated with obstructive lymphedema at the early and intermediate stages of the disease. The importance of preoperative assessment and mapping of lymphatic and venous vessels using ultrasound technologies is emphasized, as it enables optimization of surgical tactics, improves the procedure’s efficiency, and reduces operative time. The article discusses the pathophysiological features of lymphedema, including vascular sclerosis and decreased lymphatic drainage capacity, as well as the advantages of modern instrumental diagnostic methods in planning LVA.
