Оценить динамику индекса перитонеальной адгезии (PAI) и количества вовлеченных анатомических регионов брюшной полости у пациенток, больных раком яичников и перитонеальным канцероматозом в процессе последовательных сеансов внутрибрюшной аэрозольной химиотерапии под давлением (ВАХД).</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Выполнено одноцентровое наблюдательное исследование с интраоперационной оценкой спаечного процесса у 87 пациенток. PAI и количество вовлеченных регионов брюшной полости определяли на этапах выполнения 3-х сеансов ВАХД и при проведении диагностической лапароскопии отсроченного этапа. Для интервальных сравнений использовали доступные завершенные пары наблюдений. Основным оцениваемым показателем была динамика суммарного PAI; вторичной конечной точкой – динамика количества вовлеченных регионов и итоговая направленность изменений к отсроченной лапароскопии.</p></sec><sec><title>Результаты</title><p>Результаты. Между 1-м и 2-м сеансами ВАХД снижение PAI зарегистрировано у 57 (65,5 %) пациенток, отсутствие изменений – у 24 (27,6 %), повышение – у 6 (6,9 %); среднее изменение составило – 2,7 балла (p &lt; 0,0001). Между 2-м и 3-м сеансами снижение PAI отмечено у 35 (38,9 %) пациенток, отсутствие изменений – у 53 (58,9 %), повышение – у 2 (2,2 %); среднее изменение составило 0,89 балла (p &lt; 0,0001). Между 3-м сеансом ВАХД и отсроченной лапароскопией снижение PAI наблюдалось у 14 (16,1 %) пациенток , отсутствие изменений – у 70 (80,5 %), повышение – у 3 (3,4 %); среднее изменение составило 0,10 балла (p = 0,006). Итоговое снижение PAI от исходного уровня до отсроченной лапароскопии зарегистрировано у 56 (64,4 %) пациенток, отсутствие изменений – у 22 (25,3 %), повышение – у 9 (10,3 %); среднее суммарное изменение составило 3,7 балла (p &lt; 0,0001). Аналогичная направленность выявлена для количества вовлеченных регионов: итоговое снижение показателя зарегистрировано у 54 (62,1 %) пациенток, отсутствие изменений – у 24 (27,6 %), повышение – у 9 (10,3 %); среднее суммарное изменение составило 1,32 балла (p &lt; 0,0001).</p></sec><sec><title>Заключение</title><p>Заключение. Последовательное применение ВАХД ассоциировано со снижением выраженности и распространенности спаечного процесса брюшной полости, что свидетельствует о благоприятном профиле безопасности ВАХД и технологической осуществимости повторных лапароскопических вмешательств.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. To assess the dynamics of the Peritoneal Adhesion Index (PAI) and the number of involved anatomical regions of the abdominal cavity in patients with ovarian cancer and peritoneal carcinomatosis during consecutive sessions of pressurized intraperitoneal aerosol chemotherapy (PIPAC).</p></sec><sec><title>Patients and methods</title><p>Patients and methods. A single-center observational study with intraoperative assessment of adhesive status was performed in 87 patients. PAI and the number of involved abdominal regions were determined during three PIPAC sessions and at delayed diagnostic laparoscopy. Available completed paired observations were used for interval comparisons. The primary endpoint was the change in total PAI; secondary endpoints were changes in the number of involved regions and the overall direction of changes by the time of delayed laparoscopy. The statistical significance of paired changes was assessed using the sign test.</p></sec><sec><title>Results</title><p>Results. Between the 1st and 2nd PIPAC sessions, a decrease in PAI was recorded in 57 patients (65.5 %), no change in 24 (27.6 %), and an increase in 6 (6.9 %); the mean change was – 2.7 points (p &lt; 0.0001). Between the 2nd and 3rd PIPAC sessions, a decrease in PAI was observed in 35 patients (38.9 %), no change in 53 (58.9 %), and an increase in 2 (2.2 %); the mean change was 0.89 points (p &lt; 0.0001). Between the 3rd PIPAC session and delayed laparoscopy, a decrease in PAI was observed in 14 patients (16.1 %), no change in 70 (80.5 %), and an increase in 3 (3.4 %); the mean change was 0.10 points (p = 0.006). Overall, a decrease in PAI from baseline to delayed laparoscopy was recorded in 56 patients (64.4 %), no change in 22 (25.3 %), and an increase in 9 (10.3 %); the mean cumulative change was 3.7 points (p &lt; 0.0001). A similar pattern was observed for the number of involved regions: an overall decrease was recorded in 54 patients (62.1 %), no change in 24 (27.6 %), and an increase in 9 (10.3 %); the mean cumulative change was 1.32 points (p &lt; 0.0001).</p></sec><sec><title>Conclusion</title><p>Conclusion.
