Изучение состояния иммунологической реактивности у детей с хроническим гломерулонефритом.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Обследовано 288 детей с хроническим гломерулонефритом, из которых мальчики составили 64,6%, девочки — 35,4%. Средний возраст составил 10,63 ± 3,88 лет. Дети были подразделены на 3 группы: 1-я группа — 104 ребенка с нефротической формой, 2-я группа — 96 детей с гематурической формой, 3-я группа — 88 детей со смешанной формой. В сыворотке крови определяли следующие параметры: CD3+, CD4+, CD8+, CD16/56+, CD19+, CD95+-субпопуляции лимфоцитов; IgA, IgM, IgG; цитокины — ИЛ-1β, ИЛ-8, ФНО-α, γ-ИФН, ИЛ-4 и ИЛ-10; уровень циркулирующих иммунных комплексов, тест восстановления нитросинего тетразолия (НСТтест), фагоцитарный индекс и фагоцитарную активность нейтрофилов.</p></sec><sec><title>Результаты</title><p>Результаты. Иммунологическая реактивность у детей с хроническим гломерулонефритом характеризуется статистически значимым повышением Т-хелперов (на 36,2% и 41,9% при нефротической и смешанной форме), маркера апоптоза (на 50,5%, 51,7% и 65,4% соответственно у детей с нефротической, гематурической и смешанной формами), параметров гуморального иммунитета, уровня циркулирующих иммунных комплексов (в 2,4, 2,2 и 2,6 раза соответственно при нефротической, гематурической и смешанной формах, р &lt; 0,05), провоспалительных цитокинов и снижением Т-киллеров, В-клеток и фагоцитарной активности. При этом степень выраженности изменений иммуноглобулинов оказалась выше при нефротической (IgA в 2,1 раза, IgM в 2,2 раза, IgG в 1,7 раза, р &lt; 0,05) и смешанной формах (IgA в 2,0 раза, IgM в 1,8 раза, IgG в 1,6 раза, р &lt; 0,05) хронического гломерулонефрита (ХГН). Уровень ФНО-α у детей с гематурической формой превышал контрольные значения в 3,9 раза (р &lt; 0,01), с нефротической и смешанной формой — в 4,2 (р &lt; 0,01) и 4,3 раза (р &lt; 0,01) соответственно. Уровень ИЛ-1β и ИЛ-8 был выше при нефротической форме в 2,0 и 1,5 раза (р &lt; 0,05), при гематурической — в 1,8 и 1,4 раза (р &lt; 0,05), при смешанной — в 2,0 и 1,5 раза (р &lt; 0,05) соответственно. Средний уровень ИФН-γ независимо от формы ХГН превышал контрольный в 1,8 раза (р &lt; 0,05).</p></sec><sec><title>Заключение</title><p>Заключение. У детей с ХГН отмечается дисбаланс иммунной системы. Следует отметить, что само наличие хронического воспаления сопровождается нарушением иммунологической реактивности. Полученные данные свидетельствуют о гиперчувствительности при ХГН.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. The purpose is to study the state of immunological reactivity in children with chronic glomerulonephritis.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. 288 children with chronic glomerulonephritis were examined, of which boys accounted for 64.6%, girls — 35.4%. The mean age was 10.63 ± 3.88 years. Children are divided into 3 groups: Group I — 104 children with nephrotic form, Group II — 96 children with hematuric form, Group III — 88 children with mixed form. In the serum are identiﬁ ed: CD3 +, CD4 +, CD8 +, CD16/56 +, CD19 +, CD95 + -subpopulations of lymphocytes; IgA, M, G; cytokines — IL-1β, IL-8, TNF-α, IFN-α, IL-4 and IL-10; the level of circulating immune complexes, the test for the reduction of nitrous tetrazolium (HST test), the phagocytic index and the phagocytic activity of neutrophils.</p></sec><sec><title>Results</title><p>Results. Immunological reactivity in children with chronic glomerulonephritis is characterized by a statistically signiﬁcant increase in T-helpers (by 36.2% and 41.9% in nephrotic and mixed forms), apoptosis marker (by 50.5%, 51.7% and 65.4% respectively, in children with nephrotic, hematuric and mixed forms), parameters of humoral immunity, the level of circulating immune complexes (2.4, 2.2 and 2.6 times, respectively, in nephrotic, hematuric and mixed forms, p &lt; 0.05), pro-inﬂammatory cytokines and decreased T-killers, B cells and phagocytic activity. At the same time, the degree of expression of immunoglobulin changes was higher with nephrotic (IgA 2.1 times, IgM 2.2 times, IgG 1.7 times, p &lt; 0.05) and mixed forms (IgA 2.0 times, IgM 1.8 times, IgG 1.6 times, p &lt; 0.05) with chronic glomerulonephritis. The level of TNF-α in children with a hematuric form exceeded the control ones by 3.9 times (p &lt; 0.01), with the nephrotic and mixed form in 4.2 (p &lt; 0.01) and 4.3 times (p &lt; 0.01) respectively. The level of IL-1β and IL-8 was higher in nephrotic form in 2.0 and 1.5 times (p &lt; 0.05), with hematuric — in 1.8 and 1.4 times (p &lt; 0.05), at mixed — in 2.0 and 1.5 times (р &lt; 0.05) accordingly.
