Оценка эффективности метода паллиативной панкреатодуоденальной резекции при осложнениях распространенного рака головки поджелудочной железы (РГПЖ).</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. В статье проанализированы результаты лечения 32 больных с распространенным РГПЖ в отделении радиохирургии ФГБУ «НМИЦ онкологии им. Н. Н. Блохина» Минздрава России. Всем больным были выполнены паллиативные пaнкреатодуоденальные резекции (ПДР). По распространенности процесса больные распределились следующим образом: 17 (53,1%) больных с местно-распространенным процессом и 15 (46,9%) — с отдаленными метастазами.</p></sec><sec><title>Результаты</title><p>Результаты. Среднее время оперативного вмешательства составило 5,2 ± 0,3 ч, средняя кровoпотеря составила 543,1 ± 1,3 мл, средний койко-день — 22,1 ± 0,6 дней. Всего в ближайшем послеоперационном периоде осложнения были отмечены у 5 больных (15,6%). Общая послеоперационная летальность после ПДР составила 6,3%. В целом результаты были разделены на 4 группы: хорошие результаты признаны в 16,7% случаях, достаточно удовлетворительные — в 26,7%, менее удовлетворительные, но с положительным эффектом — в 50%, неудовлетворительные результаты лечения констатированы в 6,6% случаях. Кaчество жизни с помощью опросника MOS-SF-36: если средний показатель по шкале физического здоровья составил до операции 41,2 ± 0,5, то через месяц после оперативного вмешательства он был значительно выше — 68,1 ± 0,9, по шкале психологического здоровья средний показатель вырос с 30,6 ± 0,9 до 71,5 ± 0,7. При этом различия между значениями до и после операции статистически достоверны (р &lt; 0,01). Отдаленная выживаемость в течение 1 года после операции составила 39,2%, через 2 года после операции — 7,1%.</p></sec><sec><title>Заключение</title><p>Заключение. Паллиативная ПДР при распространенном раке поджелудочной железы в некоторых случаях может являться операцией выбора, позволяющей эффективно устранить основной патологический процесс и его осложнения в виде механической желтухи, дуоденальной непроходимости, болевого синдрома и значительно улучшить качество жизни больных.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. Evaluation of the effectiveness of the method of palliative pancreatoduodenal resection for complications of advanced pancreatic head cancer.</p></sec><sec><title>Patients  and methods</title><p>Patients  and methods. The article analyzes the results of treatment of 32 patients with advanced pancreatic head cancer in the Department of Radiosurgery of the FSBO «N. Blokhin National Medical Research Center of Oncology” of Ministry of Health of Russia. All patients underwent palliative pancreatoduodenal resection. In terms of the prevalence of the process, the patients were distributed as follows: 17 (53.1%) patients with the locally advanced process and 15 (46.9%) patients with distant metastases.</p></sec><sec><title>Results</title><p>Results. The average time of the operative intervention was 5.2 ± 0.3 hours, the average blood loss was 543.1 ± 1.3 ml, the average bed-day was 22.1 ± 0.6 days. In total, in the shortest postoperative period, complications were noted in 5 patients (15.6%). The total postoperative lethality after the PDR was 6.3%. In general, the results were divided into 4 groups: good results were recognized in 16.7% of cases, satisfactory enough — in 26.7%, less satisfactory, but a positive effect — in 50%, non-satisfactory results of treatment were found in 6.6% cases. Quality of life using the MOS-SF-36 questionnaire — if the average indicator on the scale of physical health was 41.2 ± 0.5 before the operation, then after a month of surgery it was significantly higher — 68.1 ± 0.9, according to the scale the average figure increased from 30.6 ± 0.9 to 71.5 ± 0.7. However, the differences between the values before and after surgery are statistically significant (p&lt;0.01). The long-term survival rate for 1 year after the operation was 39.2%, 2 years after the surgery, 7.1%.</p></sec><sec><title>Conclusions</title><p>Conclusions.
