В настоящее время для лечения данной патологии все большее развитие получают методики органосохраняющего хирургического пособия, позволяющего добиться максимального косметического эффекта наряду с соблюдением принципов онкологической безопасности. В последние десятилетия исследователи разных стран демонстрируют осуществимость и техническую безопасность использования различных мини-инвазивных методов термической абляции ранних стадий злокачественных новообразований молочной железы, отличающихся относительной простотой выполнения, прекрасными косметическими результатами, коротким сроком реабилитации больных. К ним относятся гипертермические методики (радиочастотная, микроволновая, лазерная абляция) и гипотермический метод (криоабляция). Каждая методика имеет уникальные характеристики воздействия на опухоль и выполняется под контролем ультразвука, компьютерной или магнитно-резонансной томографии. Технический успех данных методов абляции достигается в 93–98% случаев. Полный некроз опухоли после выполнения радиочастотной абляции наблюдают у 76–100% больных раком молочной железы, лазерной абляции – у 13–76%, микроволновой абляции – у 0–8%, криоабляции – у 36–83%. Частота развития специфических осложнений локальной термической деструкции составляет 4–13%. На сегодняшний момент существует лишь одна методика неинвазивного экстракорпорального выполнения термической абляции опухолей различной локализации – высокоинтенсивная фокусированная ультразвуковая терапия, эффективность и безопасность которой в качестве метода локальной деструкции доброкачественных и злокачественных новообразований молочной железы продемонстрированы многими авторами. Как и все методики термической абляции, данный метод имеет свои ограничения, осложнения и недостатки. В данной статье представлен литературный обзор, освещающий возможности выполнения данного метода локальной деструкции у больных злокачественными новообразованиями молочной железы.</p></abstract><trans-abstract xml:lang="en"><p>Breast cancer is the most frequent and socially signiﬁ cant malignant tumor disease of the female popula on of the Russian Federa on. At present, for the treatment of this pathology, the methods of the organ-preserving surgical manual allowing to achieve the maximum cosme c eﬀ ect along with the observance of the principles of cancer safety are ge  ng more and more developed,. In recent decades, researchers from diﬀ erent countries demonstrate the feasibility and technical safety of various mini-invasive methods of thermal abla on of early stages of breast cancer, characterized by rela ve simplicity of implementa on, excellent cosme c results, short-term rehabilita on of pa ents. These include hyperthermic techniques (radiofrequency, microwave, laser abla on) and hypothermic method (cryoabla on). Each technique has unique characteris cs of impact on the tumor and is performed under the control of ultrasound, computer tomography or magne c resonance imaging. The technical success of these abla on techniques is achieved in 93–98% of cases. Complete tumor necrosis a er radiofrequency abla on is observed in 76–100% of pa ents with breast cancer, laser abla on – in 13–76%, microwave abla on – in 0–8%, cryoabla on – in 36–83%. Like all methods of thermal abla on, high-intensity focused ultrasound therapy has its limita ons, complica ons and disadvantages.
