Определение эффективности антиандрогенной терапии 10-летней общей и опухоль-специфической выживаемости (ОВ, ОСВ) в зависимости от вида гормональной терапии, стадии заболевания, риска прогрессирования.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. Материалом для исследования послужил ретроспективный анализ 100 пациентов, страдающих раком предстательной железы (РПЖ) локализованной, местнораспространенной и метастатической стадией заболевания за период 2008–2018 г. г.. Оценка проводилась в 2019 году. Все больные получили антиандрогенное лечение в виде кастрационной терапии (n=59) и комбинированного (комплексного) лечения (n=41) в различных вариантах и режимах проведения на протяжении более 5 лет. Больные имели 100-процентную 5-летнюю выживаемость. Контрольной точкой исследования явилась общая и опухоль-специфическая выживаемость, а также факторы прогноза, определяющие указанную выживаемость.</p></sec><sec><title>Результаты</title><p>Результаты. К моменту окончания исследования осталось в живых 53 (53%), умерло 8 (8%), цензурировано 39 (39%) пациентов. Наиболее высокий процент 10-летней общей выживаемости достигнут при локализованных стадиях заболевания и составил 52% (31 пациент) — 1стадия, 38% (22 пациента) — 2стадия. Опухоль специфическая выживаемость (1–2стадии) — 62% (33 пациента). При 3 стадии — 30% (14 пациентов) и 51% (21 пациент) а при 4 стадии 18% (1 пациент) и 32% (2 пациента) соответственно. В зависимости от риска прогрессирования (индекс Глисона) высокие показатели как общей, так и опухоль специфической выживаемости достигнуты при низком Глисон (2–5), и умеренном риске прогрессирования Глисон (6–7), и составили 85% и 62% соответственно. Наиболее эффективным методом лечения, определяющим 10-летнюю общую выживаемость, явилось проведение комбинированной (гормоно-лучевой) терапии, что позволило добиться результата у 75% пациентов. Опухоль-специфическая выживаемость составила более 80%.</p></sec><sec><title>Заключение</title><p>Заключение. В реальной клинической практике использование антидепривационной терапии в различных вариантах и режимах позволяет получить у 75% и более чем у 80% больных 10-летнюю общую и опухоль-специфическую выживаемость. Стратификация пациентов по группам риска формирует персонализированный подход в лечении каждого пациента. Использование в работе схем лечения исследований STAMPEDE, CHAARTED позволяет увеличить общую выживаемость больных с метастатической стадией заболевания.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose of the study</title><p>Purpose of the study. To determine the effectiveness of antiandrogenic therapy — 10-year General and tumor-specific survival (OV, OSV) depending on the type of hormone therapy, the stage of the disease, and the risk of progression.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. The study was based on a retrospective analysis of 100 patients with localized, locally advanced, and metastatic prostate cancer over the period 2008–2018. The assessment was conducted in 2019. All patients received antiandrogenic treatment in the form of castration therapy (n=59) and combined (complex) treatment (n=41) in various variants and modes for more than 5 years. Patients had a 100% 5-year survival rate. The control point of the study was General and tumor- specific survival, as well as prognostic factors that determine this survival. </p></sec><sec><title>Results</title><p>Results. By the end of the study, 53 (53%) patients were alive, 8 (8%) died, and 39 (39%) patients were censored. The highest percentage of 10-year overall survival was achieved in localized stages of the disease and was 52% (31 patients) — stage 1, 38% (22 patients) — stage 2. Tumor specific survival rate (1–2 stages) — 62% (33 patients). At stage 3–30% (14 patients) and 51% (21 patients) and at stage 4–18% (1 patient) and 32% (2 patients), respectively. Depending on the risk of progression (Gleason index), high rates of both General and tumor- specific survival were achieved with low Gleason (2–5), and moderate risk of Gleason progression (6–7), and were 85% and 62%, respectively. The most effective method of treatment that determines the 10-year overall survival was combined (hormone radiation) therapy, which allowed to achieve results in 75% of patients. The tumor — specific survival rate was more than 80%.</p></sec><sec><title>Conclusion</title><p>Conclusion. In real clinical practice, the use of antideprivation therapy in various variants and modes allows to obtain a 10-year General and tumor- specific survival rate in 75% and more than 80% of patients. Stratification of patients by risk groups forms a personalized approach to the treatment of each patient.
