2020 г. деятельность онкослужбы РФ осложнилась развитием респираторного синдрома SARS-CoV‑2, что оказало выраженное воздействие на здоровье пациентов со злокачественными новообразованиями (ЗНО) и способствовало переориентированию сотрудников учреждений онкослужбы для обеспечения особого режима. Нарушена вертикаль преемственности при диагностике и лечении пациентов с ЗНО, нарушены сроки проведения лечения. Изучены аспекты организации онкологической службы Магаданской области (МагО) в условиях пандемии SARS-CoV‑2.
Цель исследования. Оценить особенности выявления ЗНО среди жителей МагО в 2019-2020 гг.
Материалы и методы. Использованы доступные источники информации (документы Минздрава МагО), базы данных онкодиспансера (МООД), основанные на сведениях из форм госотчетности – фф. 7 и 30, а также из ресурсов Интернета.
Результаты. МагО регион, лидирующий по темпам заболеваемости и смертности от ЗНО в ДФО: заболеваемость ЗНО за 2009-2019 гг. увеличилась на 23 %. В 2020 г. в МагО снизился показатель выявляемости ЗНО (с 35 % до 25,4 %), а удельный вес пациентов с ЗНО в локализованных (I-II) стадиях составил 50 %. В августе 2020 г. начал работу ЦАОП, за 5 мес. ЗНО выявлены у 35 человек (7,6 %), в том числе в ранних (I-II) стадиях у 18 человек, что существенно не повлияло на темпы ранней выявляемости ЗНО. При медосмотрах выявлено 107 случаев ЗНО (снижение на 27,4 % в сравнении с 2019 г.). В 2019-2020 гг. в МагО проведен скрининг ЗНО по ряду нозоологий: в итоге доля пациентов с ЗНО легких (МагО– 36 %, РФ– 29,4 %) и шейки матки (МагО– 82 %, РФ– 66 %) превысила среднероссийские показатели.
Заключение. Для увеличения выявления ЗНО необходимо: организация смотровых кабинетов в округах МагО; составление «карты загрязненности территории»; оценка условий производственной деятельности человека и плотности населения; внедрение стандарта обследования больных ЗНО; повышение онконастороженности и квалификации медицинского персонала первичного амбулаторного звена.
In 2020, the activities of the oncological service of the Russian Federation were complicated by the development of the SARS-CoV‑2 respiratory syndrome, which had a pronounced effect on the health of patients with malignant neoplasms (MN) and contributed to the reorientation of employees of oncological services to ensure a special regime. The vertical line of continuity in the diagnosis and treatment of patients with malignant neoplasms was violated, the timing of treatment was violated. Aspects of the organization of the oncological service of the Magadan region (MagO) in the context of the SARS-CoV‑2 pandemic have been studied.
Purpose of the study. To assess the features cancer detection among the citizens of the Magistral District in 2019-2020.
Materials and methods. The available sources of information (documents of the Ministry of Health of the MagO), databases of the oncological dispensary (IODE), based on information from the state reporting forms – ff. 7 and 30, as well as from Internet resources. Results. MagO region, leading in the rate of morbidity and mortality from malignant neoplasms in the Far Eastern Federal District: incidence of malignant neoplasms for 2009-2019 increased by 23 %. In 2020, the indicator of malignant neoplasm (from 35 % to 25.4 %) decreased in the MAO, and the proportion of patients with malignant neoplasms in localized (III) stages was 50 %. In August 2020, the CAPC began to work, for 5 months. Cancer was detected in 35 people (7.6 %), including in the early (I-II) stages in 18 people, which did not significantly affect the rate of early detection of cancer. Physical examinations revealed 107 cases of malignant neoplasm (a decrease of 27.4 % compared to 2019). In 2019-2020 In the MAO, cancer screening was carried out for a number of nosoologies: as a result, the proportion of patients with lung cancer (MagO – 36 %, RF – 29.4 %) and cervix (MagO – 82 %, RF – 66 %) exceeded the national average.
Conclusion. To increase the detection of malignant neoplasms, it’s necessary to provide examination rooms in the districts of the MagO; drawing up a "map of contamination of the territory"; assessment of the conditions of human production activity and population density; introduction of the standard of examination of patients with cancer; increasing the alertness and qualifications of primary outpatient medical personnel.
