Оценить влияние курения на непосредственную эффективность (достижение объективного ответа) индукционной химиотерапии (ИХТ) у пациентов, получающих комбинированное лечение по поводу местно-распространенного плоскоклеточного рака ротоглотки, ассоциированного с вирусом папилломы человека.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы. В исследование, проведенное с 2021 по 2023 гг. на базах Московского научно-исследовательского онкологического института им. П. А. Герцена – филиал ФГБУ «Национальный медицинский исследовательский центр радиологии» Минздрава России и ГБУЗ «Городская клиническая онкологическая больница № 1» Департамента здравоохранения г. Москвы включено 68 больных (51 мужчина и 17 женщин) с местно-распространенным плоскоклеточным раком ротоглотки, ассоциированным с вирусом папилломы человека. Больные были разделены на 2 равные группы, полностью сопоставимые по основным характеристикам (стаж курения, стадия, гистологический тип и степень дифференцировки опухоли). На первом этапе пациенты 1-й (основная) группы получили деэскалированную индукционную химиотерапию: три 21-дневных курса по схеме ТР (доцетаксел в дозе 75 мг/м2 + цисплатин 75 мг/м2); пациенты 2-й (контрольная) группы – стандартную схему (доцетаксел 75мг/м2 + цисплатин 75 мг/м2 + 5-фторурацил 1000 мг/м2). Пациенты обеих групп получали профилактически гранулоцитарный колониестимулирующий фактор. На втором этапе проводили стандартную химиолучевую терапию на фоне лекарственной радиомодификации карбоплатином. Корреляционный анализ проводили, вычисляя коэффициент Спирмена с поправкой Бонферрони. Различия считали достоверными при p &lt; 0,05.</p></sec><sec><title>Результаты</title><p>Результаты. При оценке объективного ответа на лечение выявлено, что контроль над болезнью достигнут у 33 (97 %) больных из 1-й группы и у всех 34 (100 %) пациентов из 2-й. Полный ответ отмечен у 3 (8,8 %) больных из 1-й группы и у 4 (11,8 %) из 2-й; частичный регресс – у 22 (64,7 %) и у 24 (70,6 %); стабилизация опухолевого процесса – у 8 (23,5 %) и у 6 (17,6 %) соответственно. Дальнейший анализ показал, что между курением и дифференцированным уровнем ответа (первичный очаг, измененные лимфатические узлы) нет ни прямой, ни обратной корреляционной связи; все пациенты (курящие и никогда не курившие) одинаково эффективно отвечают на первый этап комбинированного лечения, независимо от схемы ИХТ.</p></sec><sec><title>Заключение</title><p>Заключение. В нашей работе не получено статистически значимого влияния курения на непосредственную эффективность ИХТ при комбинированном лечении местно-распространенного ВПЧ-ассоциированного плоскоклеточного рака ротоглотки независимо от схемы индукции. Однако необходим более длительный период наблюдения для оценки влияния курения на отдаленные онкологические результаты.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Purpose</title><p>Purpose. Evaluation of the eﬀect of smoking on the immediate eﬀectiveness (achievement of objective response – OR) of induction chemotherapy in patients receiving combined treatment for locally advanced HPV-positive squamous cell carcinoma of the oropharynx.</p></sec><sec><title>Patients and methods</title><p>Patients and methods. Sixty-eight patients (51 men and 17 women) with locally advanced squamous cell cancer of the oropharynx associated with papillomavirus were included in the study, which was carried out from 2021 to 2023 at the P. Hertsen Moscow Oncology Research Institute – Branch of the National Medical Research Radiological Centre, Ministry of Health of the Russian Federation, Moscow, Russian Federation and the Municipal Clinical Oncological Hospital № 1 of the Department of Health of Moscow. The patients were divided into 2 equal groups, fully comparable in terms of the main characteristics (smoking history, stage, histological type, and degree of tumour diﬀerentiation). At the ﬁrst stage, patients of the 1st (main) group received de-escalated induction chemotherapy: three 21-day courses according to the TR scheme (docetaxel in the dose of 75 mg/m2 + cisplatin 75 mg/m2); patients of the 2nd (control) group were treated with standard scheme (docetaxel 75 mg/m2 + cisplatin 75 mg/m2 + 5-ﬂuorouracil 1000 mg/m2). Patients in both groups received prophylactic granulocyte colony-stimulating factor. At the second stage, standard chemoradiotherapy was carried out against the background of drug radiomodiﬁcation with carboplatin. Correlation analysis was performed by calculating the Spearman coeﬃcient with Bonferroni correction. Diﬀerences were considered reliable at p &lt; 0.05.</p></sec><sec><title>Results</title><p>Results. When evaluating the objective response to treatment, it was revealed that disease control was achieved in 33 (97 %) patients from group 1 and in all 34 (100 %) patients from group 2. Complete response was observed in 3 (8.8 %) patients from group 1 and 4 (11.8 %) from group 2; partial regression in 22 (64.7 %) and 24 (70.6 %); stabilisation of the tumour process in 8 (23.5 %) and 6 (17.6 %) respectively. Further analysis showed that there was no direct or inverse correlation between smoking and diﬀerentiated level of response (primary lesion, altered lymph nodes); all patients (smokers and those who had never smoked) responded equally eﬀectively to the ﬁrst stage of combined treatment, regardless of the ICT regimen.</p></sec><sec><title>Conclusion</title><p>Conclusion. In our study, no statistically signiﬁcant eﬀect of smoking on the immediate eﬀectiveness of induction chemotherapy in the combined treatment of locally advanced HPV-associated squamous cell carcinoma of the oropharynx regardless of the induction regimen was found.
